Your First Appointment .
Your first appointment at LifeWeigh Bariatrics will be filled with valuable information about how you can prepare for weight loss surgery. You will meet with the nurse, program coordinator, dietitian, exercise physiologist and one of the surgeons, Dr. Allen T. Mikhail or Dr. Jeffery Rosen. This initial appointment is approximately 3 hours long.
Prior to your first appointment, please complete the following:
1. Please call you insurance company to ensure you have coverage for weight loss surgery. We will assist you with the insurance process and our office will call your insurance company during your first visit to obtain detailed information about your coverage for weight loss surgery.
PPO Patients
Call the customer service line listed on your insurance card.
Ask: "Does my policy cover weight loss surgery for morbid obesity?”
If yes, ask “what are the specific criteria that are required for approval?”
Please make a note of this specific criteria.
If no, contact the Human Resources department of the employer that provides
your health insurance.
HMO Patients
Work with your primary care physician to obtain a referral for a consultation with a specialist. Your primary care physician will help you to determine if your policy covers weight loss surgery.
If you determine that your policy does not cover weight loss surgery, click here to explore financing options.

Please note: Due to high patient demand and limited availability of appointments, we have instituted a $250 no-show fee. You must give advance notice to cancel or reschedule your initial consultation.
Also, any future appointment no-shows (after your initial consultation) will results in a $25 fee.
